Policies

At your first visit you will be asked to review and sign acknowledgement of office policies. Links to those policies can be found below or hard copies can be obtained from the office. The list of documents below is not exhaustive. Please arrive 30 minutes before your first appointment to allow time to complete all additional forms.

You are responsible for meeting all office policies as described in the above forms. We have policies relating to appointments, cancellation of appointments, payment, records and conflict of interest concerns. Our Privacy Policies are derived from HIPPA regulations. Read carefully before signing.

Cancellation + No Shows

You must receive verbal confirmation from one of our staff that we received your cancellation notification 24 hours before the cancelled appointment; leaving a message on voicemail is not sufficient.

Appointments cancelled through the patient portal 24 hours in advance do not require confirmation. Appointments cancelled with less than 24-hours’ notice are subject to missed appointment charges. Charges and full cancellation policy can be found in the payment contract above.

Refill Requests

Refill requests outside of appointments are discouraged, and may not be granted if you have missed a follow-up appointment as planned during a session.

Refill requests granted outside of appointments may be subject to charges and may not be processed immediately. Please give us 2 business days’ notice.  Charges and full prescription policies can be found in the payment contract.

Copays + Billing

Copays must be paid at the time of the appointment. A $10 service charge for billing will be applied if you forget your copay, and 1% interest will be applied to delinquent accounts over 30 days. We will make every effort to facilitate insurance payments on your behalf, but you are ultimately responsible for making sure your charges are met.
