We have included several forms on this website that are often necessary to sign before your treatment can be started.

The list of documents above is not exhaustive. Please arrive 30 minutes before your first appointment to allow time to complete all additional forms, and bring the following:

• Valid ID
• Insurance Card
• Copayment
• list of all current (all medications) and prior (psychiatric) medications
• List of allergies and other health conditions

Please be prepared for your visit to this office. You are responsible for meeting the conditions of treatment as listed in the policies. If you are not able to provide the information required, evaluation and/or treatment may be delayed.
